Use this form to submit ACT Score reports, immunization (shot) records, and unofficial high school transcripts.

Documents may also be emailed to muleriders@saumag.edu, faxed to 870-235-5072, or mailed to the following address:

NOTE: In order to complete your file, an official high school transcript is required. Official high school and college transcripts must be sent electronically or by mail from the issuing institution or school directly to SAU.
