Students are also required to report if they end their OPT early. Make sure to visit the OPT or OPT STEM pages to learn more about what F1 students are required to report.<\/p>\n<h3 style=\"text-align: center\">THIS IS FOR REPORTING EMPLOYMENT CHANGES.<\/h3>\n<h3 style=\"text-align: center\">IF YOU NEED TO START AN OPT OR STEM OPT PLEASE GO TO THE PROPER APPLICATION PAGES TO BEGIN.<\/h3>\n<h2><\/h2>\n<p>&nbsp;<\/p>\n<h2>Documents needed for Reporting Changes:<\/h2>\n<div class=\"su-spoiler su-spoiler-style-fancy su-spoiler-icon-caret su-spoiler-closed\" data-scroll-offset=\"0\" data-anchor-in-url=\"no\"><div class=\"su-spoiler-title\" tabindex=\"0\" role=\"button\"><span class=\"su-spoiler-icon\"><\/span> What documents do I need to upload?<\/div><div class=\"su-spoiler-content su-u-clearfix su-u-trim\">\nThe following documents are required to when updating OPT Employment:<\/p>\n<ul>\n<li>EAD CARD- Front and EAD CARD- Back<\/li>\n<li>Employment Offer Letter on company letterhead <strong>must include<\/strong>:\n<ul>\n<li>Company name, full street address and phone number<\/li>\n<li>Employment Start Date (Start dates cannot be the last day of your OPT)<\/li>\n<li>Employer Identification Number (EIN or FEIN)<strong>\u00a0<\/strong><\/li>\n<li><em>This can be in a separate letter from your original letter.<\/em><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>STEM<\/strong> students will also need to upload the additional documents:<\/p>\n<ul>\n<li><a href=\"https:\/\/www.ice.gov\/sites\/default\/files\/documents\/Document\/2016\/i983.pdf\"><strong>I-983 form<\/strong><\/a><\/li>\n<li>Final Evaluation from the previous employer\n<ul>\n<li>This is the bottom half of page 5 of the I-983.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3>I-983 forms MUST be signed either by hand, e<b>lectronic signatures using software programs or applications, electronically reproduced copies of the signature.<\/b><\/h3>\n<\/div><\/div>\n<div class=\"su-spoiler class=\"su-spoiler-icon\"><\/span> If I am changing any information for my current employer what should I submit?<\/div><div class=\"su-spoiler-content su-u-clearfix su-u-trim\">\n<p>If you change any information that is listed on your I-983 then you must submit an updated I-983 with the new information.\u00a0 This must be done within <strong>10<\/strong> days of any change.<\/p>\n<\/div><\/div>\n<div class=\"su-spoiler su-spoiler-style-fancy su-spoiler-icon-caret su-spoiler-closed\" data-scroll-offset=\"0\" data-anchor-in-url=\"no\"><div class=\"su-spoiler-title\" tabindex=\"0\" role=\"button\"><span class=\"su-spoiler-icon\"><\/span> <strong>How do I complete the I-983 forms?<\/strong><\/div><div class=\"su-spoiler-content tabindex=\"0\" role=\"button\"><span class=\"su-spoiler-icon\"><\/span> Updating your Physical Address<\/div><div class=\"su-spoiler-content su-u-clearfix su-u-trim\">\n<p>Changes to your address can be made in your\u00a0<u><strong><a href=\"http:\/\/mysau.saumag.edu\/ics\">SAU Campus Connect page<\/a>\u00a0<\/strong><\/u>and follow these instructions:<\/p>\n<ol>\n<li>Log into MySAU and go to CampusConnect<\/li>\n<li>On the CampusConnect page hover over \u201cForms\u201d<\/li>\n<li>A drop-down menu will appear.\u00a0 Select \u201cStudent Information Change\u201d<\/li>\n<li>Follow the steps to update your address.<\/li>\n<\/ol>\n<p>It will take 2 business days for the change to be reflected in Campus Connect.<\/p>\n<\/div><\/div>\n<div class=\"su-spoiler su-spoiler-style-fancy page.<\/a><\/strong><\/p>\n<\/div><\/div>\n<p>&nbsp;<\/p>\n<h2>STEM OPT Reporting- 6 and 18 Month Validations and 12 and Final Evaluations<\/h2>\n<p>While on STEM OPT students are required to report their current employment situation after 6, 12, 18, and 24 months of STEM OPT.\u00a0 The reporting intervals should be determined by calculating how many months of STEM OPT have passed. You must use the \u201cValid From\u201d date printed on your <em><strong>STEM<\/strong><\/em>\u00a0EAD card.\u00a0 The SEVP portal also lists these dates to better ensure students know when to report.<\/p>\n<h3><strong>What do I need to report?<\/strong><\/h3>\n<div class=\"su-spoiler su-spoiler-style-fancy su-spoiler-icon-caret su-spoiler-closed\" data-scroll-offset=\"0\" data-anchor-in-url=\"no\"><div class=\"su-spoiler-title\" tabindex=\"0\" role=\"button\"><span class=\"su-spoiler-icon\"><\/span> Every 6 months you must report:<\/div><div class=\"su-spoiler-content su-u-clearfix su-u-trim\">\n<ul>\n<li>Your current address- This must be your residence, not a mailing address somewhere else<\/li>\n<li>Employer&#8217;s information (Company name, address, supervisor)<\/li>\n<\/ul>\n<\/div><\/div>\n<div class=\"su-spoiler su-spoiler-style-fancy su-spoiler-icon-caret su-spoiler-closed\" data-scroll-offset=\"0\" data-anchor-in-url=\"no\"><div class=\"su-spoiler-title\" tabindex=\"0\" role=\"button\"><span class=\"su-spoiler-icon\"><\/span> 12 Month and Final (24 Month) Reporting<\/div><div class=\"su-spoiler-content su-u-clearfix su-u-trim\">\n<p>Students are required to submit the Evaluation section of the I-983 form (page 5).
